New Patient Information – Granby, CT
Making Each of Your Visits Easy & Comfortable
Welcome to Salmon Brook Dental Associates! We’re happy that you’ve found us, and we are excited to go above and beyond to give you a truly rewarding dental experience. To help us accomplish this goal, we’re providing information that you need to know before your first appointment with us. Please take some time to review the following, and let us know if you have any questions about your visit.
Your First Visit
Whether you’re coming to our Granby dental office for your first or your fiftieth appointment, we’ll greet you with a warm smile and welcoming attitude. Then we’ll call you back, take any necessary X-rays, and perform a thorough dental cleaning. One of our dentists will come and speak with you for a bit before examining your mouth for any issues. After explaining their findings, we’ll set up any follow-up appointments as needed.
New Patient Forms
Before we get started with your first appointment, we will need to have you complete some forms. This paperwork gives us important information about your medical and dental history and your dental insurance plan. Even though we have these forms in our dental office, we prefer having you fill them out before you arrive for your visit. That way, we are able to process this information more quickly, and you spend less time sitting in our waiting room.

Although dental insurance can lower your out-of-pocket costs, you may still need some help to make treatment more affordable. Our team would be happy to help you apply for CareCredit and help make any procedure fit comfortably in your budget.
